Sunshine Coast Aged Care Placement takes the worry out of matching the best aged care accommodation facility to individual care needs. We are skilled with the placement process and will coordinate all necessary activities, giving you peace of mind.
The process begins by obtaining an Aged Care Assessment (also known as an ACAT approval). We will organise for this assessment to be completed by a group of registered health professionals, who will assist in determining eligibility for an aged care place.
Once an ACAT approval is completed, Sunshine Coast Aged Care Placement will prepare a shortlist of prospective accommodation facilities based on geographical location, care needs, financial situation, services provided and facilities requested. The short list is unbiased and will be supported by the most recent review from the Aged Care Standards and Accreditation Agency Pty Ltd. Sunshine Coast Aged Care Placement Service does not receive any financial gain from the aged care facilities.
It is recommended that a number of facilities from the prospective list are visited. We encourage application to more than one facility as the waiting list for some facilities can be lengthier than others. Sunshine Coast Aged Care Placement will organise these visits for you, allowing the valuable opportunity to view facilities, meet staff and ask any questions.
Once the decision is made, Sunshine Coast Aged Care Placement will lodge an application on your behalf. If you desire we will also attend the admission interview with you.
We will organise any necessary referrals to community services to support the individual in their home until placement is available.
